Thorough Failure Modes and Effects Analysis (FMEA) as a risk identification and mitigation tool is expensive and time consuming in a custom-engineered, design-to-order business market. In addition, there is no pre-determined way of assessing how granular the assessment should be and when to stop. Continuing to deeper levels of granularity adds costs for diminishing potential for uncovering hidden risk in the system.
